Whose problem is this?

Before acting, place ownership. Is this yours to fix, a senior's call, or the patient's decision to make? Getting this wrong is the most common SJT mistake.

Scale the response

Not every issue needs an emergency escalation. Match the weight of your action to the weight of the risk — proportionality is a skill, not an instinct.
